SHARED SCENARIO CONSTRUCTION TO EXPLAIN THE ROOT CAUSE OF A JET ENGINE DAMAGE

This loss concerns a heavy damage on a jet engine which, based on the first inspections, was originally thought to have been caused by the ingestion of a foreign object.

We performed a survey of the engine after its teardown in order to witness the damages and to determine their root cause. Unfortunately, damages to the parts were such that the survey did not enable us to ascertain the cause of the loss.

At some stage, complementary examinations in a laboratory were considered but, in the end, this is the close collaboration between several surveyors of the agency which enabled the elaboration of the sequence of event and the determination of the origin of the loss.

Results

Thanks to experience sharing between several surveyors, additional costs have been avoided and the constructed scenario was validated and accepted all parties.
